System is an IP500V2 R9.1 w/ 1 Combo Card running in basic mode (I know). 5 9508 sets and 4 analog lines.

I had this exact issue with an IPO running 6.0 in Partner Mode. I upgraded to the latest 6.0 release, upgraded to R9, reformated/recreated the SD card, defaulted and reprogrammed from scratch. Nothing seemed to work. I ended up replacing the control unit. No more reboots.

Confirm the port number. Depending on the port used (some providers use more than one), encryption may be required. If so, the IPO is not capable of encryption. Only authentication.
