transferring calls from a DS20 on IP406 drops or goes 'nowhere'

Hi,
We have an IP406 system running 2.1 and our receptionist has a DS20 unit which she uses to transfer calls along with a 2050 DT handset.
Sometimes(random, it seems) she puts an incoming call on hold, goes to transfer by hitting the appropriate button on the DS20 and then gets either nothing or when she does put the call through and then hits ANSWER/RELEASE the call is dropped.

Where can I start looking to see what is going wrong here?

When I try it, it works fine. I've started up Monitor on the VMPRO box to see what is happening but the logs only give me so much obvious info and the rest is o decipherable if only I had some log reading sheet to follow.

1st check you do not have "OFF HOOK STATION" enabled then reprogramm the dss keys using the DSS module.
 
Thanks, IPGuru. definitely not an Off-Hook station. I will reporgarma today and see what happens.
