Survey with drop down list

I have been asked to create a survey to send out to about 8 departments in my company. I have received a survey once that ask "grade us on how we are doing" and it had drop down boxes for choices, like poor, fair and good. This was all in the body of the e-mail. I have found it very confusing to some users to ask them to complete a form then save it to their hard drive then send it back to me as an attachment. Some users when they see what all they have to do, simply do not respond. With the survey in the body of the e-mail, they simply choose from the drop down boxes then hit reply. Problem is I have no idea how to get this in the body of the e-mail. Can someone tell me how this is done? Thanks, Larry
 
Use Voting Buttons.

Create a new e-mail and open the properties.
Under Voting and Tracking options you'll see Use voting Buttons:

Type in all of the different options and separate them with semicolons (;)

Each option will appear as a command button on the sent e-mail. When the user clicks on the appropriate command button the response will be sent back to you.

You may want to send one to yourself first to make sure its OK.
