Is it possible to set up a user rights group to require an account code for long distance calls while allowing 800 and local calls? Also, what effect does an account code have on 911 calls?

Short Codes to bypass account code for local and 8xx calls.

All of this can be achieved in the ARS. What I did was check marked the Force Account Code for the 1N 0N dialing short codes in the ARS. For your example, you will need to take it a step further. For all the 1N calls, you need to add 1800N and uncheck Force Account Code.
The 911 calls should not have the Force Account Code checked or you will be in deep trouble!
