Outlook 2000 - Form Design - Customize subject line?

Hi,
I'm working with a form in outlook & I'm having some trouble customizing subject line. Currently it Just says Notice. This means that all the messages posted in this folder just have the subject of "Notice." I'm trying to change it so that the subject is [01-Name] - Notice.

This way they will be able to sort the folder by the the Name they enter in on the form (field name is [01-Name] ).

Anyways in an access query it would look something like [01-Name] & " - Notice", but I can't figure out how to do it in outlook for this form. Everytime I play around with it, it oddly will spit out the date in long format for the subject line? Any ideas?

Ok I've made some progress & I changed what was going to be in the subject line... This is what the value is for the subject line & is working (almost!)

[03-Date] & " - Notice"

03-Date is a field they have to enter, it's default value is "none" so when the form opens the subject line will be
none - Notice .. until they enter a date in which it will automatically change itself to whatever is entered (sort of).

The only thing wrong is it's adding 8:00:00 AM to the subject line.

So if someone enteres 12/7/2002 in the field 03-Date, the subject line will be

12/7/2002 8:00:00 AM - Notice
but the 03-date field will only say 12/7/2002 (it won't add the time, which btw is wrong since my cpu clock says 12:30pm).

The date field format is 12/7/2002; I don't have the one with the time selected. Additionally, I've tried Value & Text for type of property to use (same result).

I think I can probably do a trim function, but I'd rather I just set an option and it work 100% the way it's supposed to. Any Ideas?
