Mis dialed digits on Norstar Modular DR5

The unit is running version 3.X software (second hand info.) Has anyone experienced misdialling of digits? Ex: 1234 dials 1223 (2 gets replicated). This happens sporadically and on different handsets which rules out a keypad issue. If anyone has seen this, could you share what was done to rectify the problem.
A power cycle has been done to the unit.

whats DR5 and whats version 3.x
There should be only one version per system.

assuming you have a CP, there were DSP patches to fix dtmf recognition errors

I wrote version 3.X because I'm awaiting confirmation of what is actually on site. Ex: 3.1, 3.2, etc...
DR5 is a description I saw on a manual that is in our shop. I was told it was part of the model of this particular Norstar.
Sorry. This is definitely not in my comfort zone.
