Log Shipping out of sync

My log shipping monitor shows out of sync alerts. Seems
last_copied_last_updated, last_loaded_last_updated fileds in the log_shipping_secondaries not getting updated. I updated these fields manually with latest date but still it is showing out of sync. I increased out of sync threshold with higher value, still problem not resolved.
How do I make sure sqlmaint.exe should update the tables?

Lets get a bit of info first. In hearing that you are using the Maintenance Plans for log shipping I can deduct you are using SQL Server 2000 and not 7.0

Where is your Monitor located? On a seperate server as advised by MS or is it on the Warm/Stand-By server you are shipping the logs to.

Did the Monitor correctly show the log shipping events ever or does it still show the initial log being shipped and nothing else?

I am using SQL Server 2000. Monitor server is on standby server. Backup, copy, restores jobs are working fine without any problem. Transactions are being applied correctly. Only alert job - restore fails and gives Error: 14421. out of sync of 18000 minutes.

Log_shipping_secondaries table not getting updated. I have system administrator privileges on sql service account.I applied sp3a on monitor server from there it gives this error.
 
I resolved the issue by changing log_shipping_monitor table's monitor_server_name field. It was primary server's name in this table i changed back to monitor server's name and it is working fine now.
