IP 500 Embedded Voicemail Incompatible

I have an new IP 500 Control Unit with the embedded voicemail card. When I hit the voicemai access button I programmed on the 4412D phone the display says IMCOMPATIBLE Disc with a constant DTMF tone. I went to System Status and it says congestion count 20 going up 1 every time I try to access it. I know my short code is correct and I have powered the unit off and reseated the card still no luck. Any help would be appreciated.

What does it say in System Status under the System screen. Should be an entry for "Compact Flash".

Although it could be that there is something wrong with the card, have you tried another?
 
4412D is on an expansion module? does it work from a 5410 phone in the same port?

I am having an issue at several store locations where if a caller hangs up while the hold reminder is alerting at a phone then the hold reminder rings FOREVER. This is happening on IP406V2 systems that are on 4.1(9) and 4.0(7).
I am using 30 and 16 port DS modules with Merlin 4412/4424 phones. Not using more than 4, 4424 phones on any one module. I have tried resetting the phone by unplugging, soft reset, and a full power down of the system and as soon as the system is back up the hold reminder starts alerting again.
I found an Avaya Bulletin this but doesnt make sense because I have already upgraded these sites. (its the last bullet point) Thanks any help would be appreciated.

6 Issues Resolved in IP Office 3.1 Software
IP Office 3.1 software has parity with the IP Office 3.0 August maintenance release.
As well as containing all of the fixes in that release the following issues are also
resolved.
6.1 Core Software
• Noise on auto-recorded calls that originate from a cell phone.
• System resets if it receives a Disconnect Code of 81 or 91 from the ISDN
provider.
• Reboot when using L2TP with Microsoft Windows 2K as the client.
• Using call listen on a call that is already being recorded causes system to
freeze.
• If a held call is hung up while the hold reminder is alerting at a phone then the
hold reminder rings forever.
