attach terminal (note the DB9 has pins 2-3 opposite to Extreme)
while powering on the switch, hold down Ctrl-C
wait for a few bootup lines, and you will be presented with a few one letter commands.
choose "i" and answer yes. this erases the config.
select "a" to continue bootup. Ctrl-Y to login. You now have factory defaults for all settings.
Extreme password recovery
unplug the power and plug it back in with the space bar held down on the console. If you miss the boot menu, try again, it is a very fast process.
Once you get the "BootROM-> " prompt, enter "h" (for help) and you will see your options.

k: Erase selected configuration
p: Boot PCMCIA card
s: Load code image from serial port using XMODEM

Type in the option for "Force default configuration" and then "Boot on board flash"
Restore the configuration from the tftp server.
