Hold Reminder Constantly Ringing

I am having problems at several sights where if a caller hangs up while the hold reminder is alerting at a phone then the hold reminder rings FOREVER. This is happening at sites that have IP406v2 running 4.0.7 and 4.1.9. They are using 4412 and 4424 phones but not more than 4, 4424 phones are on any one system. Nothing stops this from ringing other than unplugging the line from the trunk module. I have tried resetting the phone, remote reboot of the phone system, hard reboot on site and nothing will stop it from ringing.
I found this in tech bulletin 52 but doesnt make sense since they are on a higher version than this. Any help would be appreciated. (see last bullet)

6 Issues Resolved in IP Office 3.1 Software
IP Office 3.1 software has parity with the IP Office 3.0 August maintenance release.
As well as containing all of the fixes in that release the following issues are also
resolved.
6.1 Core Software
• Noise on auto-recorded calls that originate from a cell phone.
• System resets if it receives a Disconnect Code of 81 or 91 from the ISDN
provider.
• Reboot when using L2TP with Microsoft Windows 2K as the client.
• Using call listen on a call that is already being recorded causes system to
freeze.
• If a held call is hung up while the hold reminder is alerting at a phone then the
hold reminder rings forever.
 
is that on analog lines or PRI, if you have that on analog lines then have the line provider put disconnect supervision on the lines and it should release the lines when the other party hangs up. If you have that on a PRI then I am sorry I have no idea why.

These are loop start analog lines. Don't most line providers automatically put disconnect supervision on the lines? I didnt know this was something you had to specify when ordering? Thanks
 
Upgrade to what? They are already at 4.0.7 and 4.1.9 and the tech bulletin 52 says that upgrading to 3.1 would fix this problem. Thanks
 
4.2 is what I imagine he means, 3.1 would be a downgrade.
