Hi, I have and yes it needs to first go to R11.1 as the Avaya instructions.As the title states,
Has anyone upgraded the V2 straight from 11.0 to 12.1? Any issues? Or does it need to be stepped from 11.0 to 11.1 and then to 12.1?

I could have sworn that V2 standalone can go straight to 12.x. It is only Servers that need to be stepped.No problem, try not to do short cuts with Avaya, could get issues on the long run.
