Check to see if your using auto-in for call, if so the headset needs to be turned on. to get around this switch the station form to manual-in. Wisdom is Knowledge

I myself faced the same problem with 6408D+, This happened when I changed the call master with a 6408D phone & changed the phone type in station form & started having the similar problem.
I tried doing everything but of no help, The last option I did was to remove the station & added it again & all worked perfect..
Try this once & let me know if this helps.. Have you also checked loggin-in with a different ID?
I too have had this problem. It turned out to be the auto answer feature on the login. When it was set to ACD it worked but only on a callmaster or on an analogue phone. When I changed this setting from ACD to STATION it allowed the agent to login to the 6408D.
