I have had 6416D+M model phones that had integrated headset jack. The only problem I encountered with those is that we do call recording and have external beep tone generators attached to them and the beep tone would not work through the 6416D+M headset jack, but other than that it's a great phone.
I haven't seen any. We use Unex headsets and their amplifier has a short line cord which plugs into the set's handset outlet while the handset cord plugs into the other outlet on the amplifier. We then set up a button labelled "Headset" (typically the bottom button on the set). (On a 6402D set, we use Feature code 0 for "headset".)
Pressing "Headset" (or Feature 0) toggles the headset on and off.

Checkout Avaya's website for the 6400 series product summary. There is a table about 1/2 way down the page which has the feature comparasons. For the headset jack, you will want to look in the column that says "Application Module Ready" the last column on the right.
