Rob,
Hosting an application can actually increase profit margins in the long run. While the start-up cost of doing it yourself can be cost prohibitive you can always co-locate your application at an ASP.
I recently refered a client in a similar situation to infonectASP.com. With this company...
citrix presentation server 3 automatically sets the "run only published applications" checkbox in the Citrix connnection configuration utility.
Is the desktop actually published or are you trying to get to it via a custom ICA connection?

I've used a USB to serial adapter to sync to MPXP via a thin-client.
Like you it was only for a few users, it worked great...

Publish the application Twice
ApplicationInstance1 to group1, set "limit instances allowed to run in server farm" to 2 or 3
ApplicationInstance2 to group2, set "limit instances allowed to run in server farm" to 2 or 3
Now you'll be restricting based on group.
Should work like a charm...
I also want to look at using SQL rather than Access. How would I move the datastore from Access on one server to SQL on another server?
DSMAINT MIGRATE
From where do I install SQL?
THE CD?
Also, does a SQL datastore stay on one specific server like Access, or can it be replicated to more...
This is the part that really worries me:
"Regardless of disconnection or other users or even machine name (though we've tried to keep them unique)"
You MUST keep the client names unique. While I'm just guessing I'd bet those effected by this hijacking have identical ICA client names.
This is...
Typicallt you either publish a desktop OR publish apps to a group. If I read your post correctly you want to do both?
For the desktop you essentially have two choices:
1. Windows policies to lock down the desktop and only allow access to specific applications.
Details of how to do this vary...
This should be a web server issue and not a Citrix one.
Depending on your server type you have to configure it to use the cert.
Check verisign.com they have quite a few docs on how to install and configure certs (regardless of where you got them)

You could give %SystemRoot% a shot. I just tested it on a 2003 XPFR3 box and it worked like a charm.
Example:
%SystemRoot%\system32\notepad.exe

It sounds like your creating "custom ICA connections" in PN.
To get all of the published apps try this in PN:
"Application set manager" / "Find new application set" / WAN /
Select your farm (if unavialable use the server location button to add the name or IP...
To determine if it is a file permission error:
1. download and run Filemon on the server:
http://www.sysinternals.com/ntw2k/source/filemon.shtml
connect as a user and look for "failure"
apply the needed permissions to the file.
Note that it could be a registry/permission problem...
It soulds like Nfuse and or Metaframe is configured to return an Alternate address. The problem is that it's returning the Old address and not the new on.
To verify:
when you see the application Icons in Nfuse right clik one and "save as"
Open the launch.ica file with notepad and if...
since your getting the "citrix server cannot resolve name" error it souds like an SSL/DNS error.
SSL requires FQDN's.
If your outside users are using someone elses DNS they may not be able to resolve the FQDN of the server.
Since you haven't configured CSG yet did you select SSL/TLS...
If your using a local access DB (mf20.mdb) you'll need that file and the DSmaint command. It's much easier to move i your database is resident on another server (SQL).
Details on dsmaint can be found in the advance admin guides.
