Wipe Phone Clean

We are using IPO500 with 5610 phones. What is the procedure to wipe a phone clean (completely clean) of its previous settings, especially the extension that was once attached to it. On a couple of phones, IPO System Status displays old extensions under 'Home Extension number' but lists current extension under 'current extension in use'. Also, is there a sheet somewhere that lists all the commands that can be input from the phone itself? I have looked high and low in Avaya's website, but that is a joke. BTW, I have tried 'hold+clear'; 'hold+reset###'. None of them wipe it clean.

Leave the handset on the extension, make sure headset is NOT turn on while pressing HOLD C L E A R #

Look on the Display, does it give you the options Yes or No?

I am pretty sure I have cleared the phone with the handset on, but will give it a try again.
 
Or press the option button and go to log off
This will let go the extension too (probably from 4.1.15 up)
