Firewall.

Does the built in 2k3 firewall provide the granularity to allow incoming 1433 from a specified list of hosts and block all other tcp/udp 1433 requests?

I'm not seeing that it does. But I'm a little gun-shy after having installed a pro version of zone alarm on a web server and seeing it brought to its knees once...

Any recommendations on good and cheap commerical firewall apps?

Why bother? You can get a reasonable firewall for under £60 now. Generally better than any software solution other than products like ISA server and Linux boxes!!!
If your in the UK try
Got something simlar for my home office network (wireless and from 3Com!)

Unfortunately its a colo server, so we really need a software firewall we can install remotely..
