Voicemail stops responding

I have a IPO system that is now currently on 9.1.11 that after a few hours to a day the voicemail simply stops responding. It is embedded voicemail and system status shows the voicemail is actually active but if you make any call to voicemail, dial shortcodes for the AA, or hit the message button it just rings and never answers.

What we have tried so far:
Reformating and recreating the SD card(both of them actually)
Replacing the SD card
Versions 9.1.6, 9.1.9, 9.1.11
Replacing the control unit
Rebuilding the config from scratch

I actually could replicate this issue on my test system, but only on 9.1.9, and only if I randomly unplugged power while running. I upgraded to 9.1.11 and my test system stopped having this issue even when improperly shut down.

If anyone has any idea what might be happening I would be internally grateful as this has been going on for a while and I am simply out of ideas on what it could be at this point.

The truth is just an excuse for lack of imagination.
 
I had a very similar issue with one of our recent R10 installs. Ended up convincing Avaya to replace the whole thing.. control unit, cards, and SD card. Only then did it actually resolve the problem permanently. Sounds like you've done everything but the cards inside the IPO.. I can't fathom why that would cause a problem but you never know.. Do you have spares that you could put in?
