User defined forms with printers 1

Hi all,
I could not find the "user defined form" entry among the Printer Forms entries. (searched for several printer types)
I am missing something ?

As I stated before in "searched for several printer types"
I defined several printers and could not find "user defined form" entry for any of them.

File, Server properties...

The customization for forms is on the first page.
