Tab Controls & Field Names on a form

I have what will seem like probably a dumb question but I have had very little experience with tab controls on a form. My data consists of a list of customers and their dependents, birthdates, etc. Below is an example of the primary fields on my table, and in the case of a family of 4, there will be 4 lines. The contract holder (subscriber) will also be a member on the first line, and the Spouse and 2 Children will each have a line where the Subscriber info remains the same but the member info is specific to each of them.

Address City State Zip
Member MembBirth MemRelCode

My 1st tab is titled Subscriber and will carry his/her info.The 2nd tab is Members and I want each members information there. How do I get all members to show if the field names are the same in the case of each of 4 members?
