Anybody know how to get the Stored Procedure Debugger in VB installed and running?

I have read an article in SQL Server mag about the SQL Stored Procedure debugger in VB6.0. To invoke it, you are supposed to go to the Add-Ins menu and select it from the list.

In my Add-Ins menu, it is not listed. In the Help, it states that it gets installed with the 'Enterprise Tools'. I re-installed Visual Studio 6.0, and included the Enterprise Tools. Still not showing up in the Add-Ins.

Anybody know what you have to do to get it installed???
