Are you asking for some client side add-on or some software to run on a mail server? If you want a completely hands off approach, I'd recommend postini.

For server-based spam fighting, GFI Mail Essentials and Mail Security is outstanding.

For server based spam filtering check out XWall. We've been using it for 10 months and now have it configured to stop 97% of spam and 99% of viruses before they hit Exchange.
