Setting up radio buttons in excel form

I'm setting up a monthly eval form for our department. I'd like to set low to high 1-5 and have a radio button in each column to click on. Obviously 5-best 1-worst. There are 10 questions total.

First question, how can I put a rdio button in each cell w/out having to make a new one each time and resizing?

Second question, I'd like to have each rating add up to 1 grand total %, sort of like a test to give each person their score.

Bonuses will then be based on these scores, who scores best out of the 4 individuals in our dept.

If this doesn't make sense, let me know.

Forms, then the radio option button. I don't like the check boxes.
 
The operate differently.

Format and size one.

Link to a cell

Then copy 'n' paste.

Group 5 within a Group Box

Copy the group box et al 'n' paste in a new row

edit the cell link

repeat 8 more times
