Service Observe Echo Issue

We recently installed a S8720 CM4.0. Since the upgrade, when managers try to service observe a call. There is an echo. The supervisor and agents only hear the echo. The firmware is updated. Any ideas?
 
what version of CM4 are you running and which update is installed?
What firmware levels are you running on IPSIs and Medpros
do a list trace on the obvserved station see if this shows anything

there seem to be a lot of known issues on CM4 relating to service observe, I would think about contacting Avaya
