RJ11 patch cable for IPO installs

I'm trying to locate premanufactured (and tested) cables that are 1 pair terminated on an RJ11 mod plug that can be punched down on 66 or 110 block or 4 pair to 4 rj11 plugs but my internet search keywords must be off. Does anyone have a manufacturer and/or part number for this item.
