"If you like to make a call...." prompt from carrier.

I'm sure we all have encountered the infamous "If you like to make a call please hang up and try again. If you need help dial your operator" prompt. A high pitch sqeal is hear just prior to the prompt being played. I'm curious as to why a caller would encounter this. I hear this at times when I'm being transferred to a destination from an IVR application.
 
I've had this happen to me when I've taken too long to enter all of the digits of the telephone number that I'm dialing. The attempted call will time out and I receive the recording that you describe.

what I beleive is happening is the IVR transfred you to a second line to make the call to another location and then failed to dial or dialed before dialtone was received

I have had that happen to me and if you hold on you eventually get the high pitched co warning tone trying to get you to hang up
