Public Folder Question...setting up a form for all users

I’m looking to create a public folder that allows my users to access a pre-designed order form. I have an idea in my head as to what it should look like, but for the life of me cannot get it to function. Its basically a task that when clicked on opens a word document where users fill out information, when its saved it places it like a normal task, with levels of priority, completed or in process. The problem I am having is attaching the word document, how would I go about doing this? Would it be attached to the actual folder, or does it attach to the first line? Or is it something all together different?
