Problems Exporting to a .DBF file

I am trying to create a .DBF file that will contain a "Code" and a "Description" field. How do I present this information in the report so that it will create the proper Headings for the fields and data in the proper place?
 
Sort by date Sort by votes
If you report just has those two fields and nothing else, it should export fine in the order you have them.
