Problem with File Security

I have one user who is having trouble accessing some of his files. His files are on the server and in specific file folders he created. All of a sudden, he can't access one or two files out of a folder. A week later, he can't access a different file out of a different folder.

I checked the security of the folder and the permissions say Administrator and him. The individual folders underneath say the same thing. It's just on individual files where his permission is gone and it's just Administrator and something like: S-1-5-21-854245398-861567501. What is this thing and how did it get there?

When I select it and click remove, i get the message saying "You can't remove because this object is inheriting permissions from its parent. To remove it, you must prevent this object from inheriting permissions. Turn off the option for inheriting permissions, and then try removing it again".

I unclick "allow inheritable permissions..." then click remove. Then I add the Admininstrator and the user back to the file.

Can I prevent this from happening?
