We have had a couple sites do this and I have heard that this has occurred with a number of our competitors systems as well.
I don't have an answer to it the only thing we can put it down to is the speed the IPO reboots the exchange fails to sync again. What we do is remove the PRI for 1 minute and this gives the exchange time to time out then plug it in hopefully up she comes.
