Phone Register with Wrong TFTP

Hello,i'm working with CCM5 for some reason the phone doesn't see the CCM ip address as TFTP instead uses the voice vlan IP address. any suggestion. also on the gateway keep on getting Translating "host name" i think is a domain issue. thanks in advance.
