Outgoing only trunks looped together

Hi guys, I did a search and only saw one related thread "Tie Trunks Looped" with one response that they may have been conferenced and not released or call forwarded to 9 and not released.

I seem to have too many to blame on conference calls or call forwarding and they are locking up my channels until I disable one of the channels and the other becomes IDLE.

Rather than trying to fix my looped problem (although I'm open to suggestions) can someone explain the LD 80 trace report?

I don't understand how a conference call wouldn't show the second party number, or the originating DN.

So Phone "A" dials 9-1-440-XXX-XXXX (Outgoing loop 202 channel 13) and then presses CONF, dials 9-1-XXX-XXX-XXXX and conferences the two parties together.
Wouldn't I see a separate trace line DIAL DN for let's say Loop 202 Channel 16 with 9-1-XXX-XXX-XXXX ? Why am I not seeing Phone "A" DN like this normal trunk call -

These are outgoing trunks only, no DID coming in and then connected to an outbound trunk.

How are two trunks BOTH connected to the same DIAL DN?

I'm assuming CALL ID 0 is referring to Customer 0? Is the next number simply a reference number or transaction number 3238, or 10554?
