! Out of sync audio

What would cause my audio to be out of sync with my video when exported?

I have tried several different settings but it's still oos.

i have noticed that differing format (save for web) outputs
are slightly different.
i went to "compensate" by separate the video-audio tracks to shift to be slightly out, and the one web format was right on but the others went out farther.
go figure...
i'd be curious to hear how other people solve/experience this
