Out of Office message not being received by all

A client stated that he set his Out of Office assistant, but some people stated that they never received his Out of Office message. From what I understand once a person receives an Out of Office response, and they send additional messages to that recipient they won't get additional responses. I explained this to my client and he stated that they never received a response. Please help.
 
Is it possible that someone sent a message to "all" and Exchange replied to the sender, but now thinks it has sent the message to "all"
 
Thanks to those who replied. I work at a help desk, which has multi-leveled support the lowest level is 1 and the highest is 5. I'm first tier support. After reviewing the above suggestions and determining that the client didn't use Outlook Web Access I contacted the Exchange Sever admin and was told that the Out of Office reply to the Internet was disabled.
