One way audio from SIP extension on outbound external calls only

We are having an issue with one of our SIP extensions that uses a Lifesize phone where on outbound external calls initiated from the Lifesize no ringing or audio is heard through the Lifesize phone when the call is made and answered. The called party can hear the caller speaking, but not the other way around. On internal calls, however, from the same Lifesize phone two way audio works perfectly. Also, inbound external calls to the Lifesize phone work without issue, the problem only seems to effect outbound calls from the phone.

I've tried replacing the Lifesize phone without an issue and we have another Lifesize system connected to our IP Office as a SIP extension that works correctly. We are running IP Office 9.0.2.

Can anyone shed any light on why the calling party does not receive audio on external outbound calls only?

Have you tried running STUN? Very often SIP or IP phones behind a router or firewall get confused as to where the RTP traffic should be sent and it ends up getting dumped to a gateway or another address and lost. Run STUN on the IP Office and see if that helps.
 
I haven't tried STUN. If the phone isn't going through a firewall to reach the phone system is STUN needed?
 
Disable Direct Media Path on the extension.

I tried disabling Direct Media Path and the same issue persists.
