Number Busy?

I have a 4.1.12 406V2 with a PRI running 5ESS protocol here in the states that's having a strange problem.
At this location all calls ring in to the receptionist(this place only has 1 phone number!)and she transfers the calls accordingly.
The issue occurs when the outside party disconnects the call before the end-user has a chance to do so.The phone displays "Number Busy" and a solid tone is played through the phones speaker.Using the drop button clears it but it is quite annoying to say the least.I saw a setting under the system tab about disconnect tone and set it to none but that had no effect.
Is there some new parameter on the IPO that I don't know about?
 
I had somthing similar to this but, what happend was when someone called into the recptionist the call would automatically go to speaker..

What I found was a user short code for the receptionist that somehow got created during the upgrade I did to 4.1.12. I deleted it and evrything works fine now..

Possible something similar.. Check short codes for receptionist..

Try to activate "Off hook Station" on the User.
 
I actually ended up sending traces to our distributor who escalated this to Avaya.I'll post the results when I get them.Thanks for your time and effort folks...
