Well Performance review time is here and my manager wants my feedback on my eval from. ie: write my own eval he knows very little of what I do. I'm really bad at this. Anyone know of a site on the net that has canned/example eval forms for our type of work?
Examples of headings I have to respond to:

Technical Expertise
Decision Making
Drive for Results
Adaptability
Communications
Manage Execution
Focus on the Customer
Demonstrate Team Membership
