No outbound calling via Cbeyond SIP

I have an IPO 500v2 running 9.1. Cbeyond is the SIP provider and the IPO is registered and inbound calls are working fine. I must have something wrong with my short code or ARS but I have no idea what. Can someone please take a look and help me out?

I have attached the log that shows authentication failure, why would it fail on an outbound call but not for registration?

Well, since there is neither the INVITE or REGISTER packet in the log you provided I can't know, I you look in those maybe you'll see what's wrong.
