basically a customer of ours phones someone and once they've been put on hold they get their own MOH playing back to them? has anyone seen this problem before??

No traces required. Read back a couple of messages. The observed behavior was attributed to "One of our engineers left a customer holdmusic.wav in his folder and then every site he went to got it uploaded".
