When it doesn't work, what's the status of the alerter service?

Start, Run, Net STOP messenger
The Alerter service is not relevent here. The only other way to do this is to close TCP and UDP ports 135, 136, 137, 139 and 445. This also shuts down networking if applied to the LAN side of your traffic. However if applied by your firewall to the WAN side of things it stops Messenger SPAM.
