At the end of the vxdisk list output, is there an error record for the failed disk, something like "diskname ... failed: was Disk_22"?
Was it definitely in a Veritas Volume Manager disk group before it failed?
Have you been able to confirm whether the disk has really failed or not? Under what circumstances did it "fail", i.e. was it just disconnected temporarily or something?
If the disk is working fine again, vxdisk scandisks (or vxdctl enable on older versions of VxVM) should change the disk's state from "error" to "online". Then a vxreattach -br should reattach it to the original volume.
