Need Help Designing an Outlook Form

I want to create a form to be used in a Public Folder to add a special type of order that needs to be tracked. I figured the easiest thing to do was to crate a "task" folder and then to design a new task forms with the fields I wanted in it. Unfortunately after creating the folder, I can't seem to redesign the form. (I've never done this before.) When I create a new form based on the task form, I cannot add fields to page one of the form, and the "design this form" button is grayed out. I tried creating another form based on the "post" template, but when I try to drag task fields (like "assigned") onto it, I get an error saying the field isn't compatible with the type of form I'm designing.
