Did the music on hold ever work? Have u checked to see if the music source works independant of the pbx?

We've had problems with overhead paging / MOH (music on hold) a few times and each time it has been one of the Sneak Current Protectors - grey wall-mounted box. they are a very old design and can fail after use.
another thing to check with the MOH provider is grounding. a simple loose cross-connect can be enough to throw things out of wack.
