Multiple Upgrade Failures 8.0 to 11.1 SP2 FP4

This past Friday I was upgrading an IPO 500V2 from 8.0 to 11.0.4.1.0 Build 11 . This is something I have done many times before without issue.

The first indication I had that this was not going to be a good day was that I could connect to this IPO with 8.1 (95) Manager, the bottom left of the Manager window said "Loading Configuration", then "Validating Configuration" but I could never see the configuration, the screen was blank.
I used that 8.1 (95) Manager to successfully recreate the SD card as the first step of the two step upgrade. When I reinstalled the SD card and rebooted the IPO it was not upgraded to 8.1 (95), the IPO was still at 8.0. WTF??

I attempted to use that same Manager 8.1 (95) to perform the upgrade through Manager (File - Advanced - Upgrade) but I received an error immediately stating that the card could not be read.

I used a different laptop with Manager 9.0 to attempt an upgrade to 9.0 instead (File - Advanced - Upgrade) and this time it worked. I successfully upgraded this IPO to 9.0. Hooray!!

I uninstalled Manager 9.0 from that laptop and installed Manager 11.0.4.1.0 Build 11 and attempted the second upgrade (File - Advanced - Upgrade), everything was going fine until about 25% of the way into the copying of over 4000 files. I got an HTTP error that stopped the file transfer and then a pop-up saying "All Systems Upgraded Successfully" - Bullshit!!

I attempted to use that same laptop and Manager to recreate the SD card, but received an error that stated some obscure .jpg file about 10 folders deep was missing from Manager and the card could not be recreated.

I copied the .cfg file to my desktop and formatted the SD card using Manager

Tried to recreate the card again but received the same error about a missing .jpg file

Uninstalled Manager from that laptop then reinstalled it.

Tried to recreate the card again and this time it worked - Hooray!!

Copied the .cfg file onto the SD card and reinstalled it, rebooted, and now after 7 hours the IPO is finally at 11.0.4

Used the PLDS Host ID to generate the upgrade license and successfully loaded it.

Re-recorded the auto attendant greetings (Embedded VM) with no problem

Tried to make a simple change using Manager but when clicking on the "Save Configuration" icon the "Save Configuration" window does not open. I am not currently able to make any changes to this IPO.

I'm at a complete loss, the only thing to do now since it's Friday afternoon and this IPO is working is to go home and have more than one drink.

Since the SD card was formatted and recreated, and given all of the other problems I had with this upgrade I'm thinking the IPO cabinet is bad somehow. It's the only thing that makes sense.



 
I had a number of issues with upgrading to R11.4 SP1 from the website, Avaya gave me the release 11.0.4.1.13 build 1 which has been working for me for all upgrades so far. It is a private build so you will need to contact Avaya for it.
Also was it the avaya_POW.jpg that was missing or something similar to that?

I had an issue with R11 SP2 F4 as well with failing to upgrade. I also couldn't recreate the sd card either. I had R10.1 on my laptop as well and even though the upgrade was pointed to the R11 folder the R10.1 was being accessed. I deleted R10.1 and R11 and then reloaded just R11 and it went through. Not sure if you have multiple Managers on your laptop, but that seemed to be the issue for me even though they were in completely different folders and I have had it working like that for years.
