MPLS ? The mpls provider is asking for me to fill out the following form Help appreciated!

I am setting up 3 sites. Site A will have IP office R9.1 and PRI using lan 1 customer network and wan for mpls ip 192.168.42.1. Sites B&C will have IP phones connected to site A using a mpls connection. Sites B&C the mpls is voice only. Site A the customer will be using the mpls router connected to firewall for backup internet. The mpls provided me with a mpls setup page. I have never had to fill out a form like this before as previous co I worked with I told them what I was doing and they just setup. The asks the following
1- Will you want the internal networks passed through a customer-provided firewall or router? I will not be installing a firewall at site B&C.
2- Please provide the LAN (Local Area Network) subnet. I plan on using site A 192.168.42.0 Site B 192.168.43.0 Site C 192.168.44.0
3- Please provide the LAN (Local Area Network) netmask. I plan on using 255.255.255.0 for all
4- What will be the IP address assigned to the Clear Rate router? I will use Site A 192.168.42.254 Site B 192.168.43.254 Site C 192.168.44.254.
5- Is there a default route for MPLS traffic? This is one ? I have. Should I have site B&C have default route to ip office wan port 192.168.42.1? If not any suggestions?
6- Will port forwarding be required? This is another ? I have. If this is a mpls why would I need forwarding? If I need forwarding is it the ip office ports?
7- Would you like Clear Rate to provide DHCP? If I click yes it ask for primary and secondary dns and domain name
I would greatly appreciate any insight!
 
Any thoughts on this. I would appreciate it!
