Mitel SX-2000 dial tone but can't dial

I have a room phone that when I pick it up I hear dial tone. When I start dialing, I hear the beeps from the keys, but the dial tone never goes away, and I can't call any extension or outside. any ideas? I tried a couple different room phones.
 
First thing I would suspect is a bad DTMF receiver, which lives on the peripheral switch controller for that cabinet. Check to make sure the trouble is isolated a single peripheral cabinet, and if so, replace the PSC card.
 
oh gosh, we just replaced the ONS card... should I try a power cycle of the cabinet?
 
I'd try a graceful re-load of the PSC first ("load <cabinet> <shelf> 16") first. But again, make sure the trouble is limited to a specific cabinet, otherwise your problem lies elsewhere and none of this is likely to fix anything.
 
It appears to be just this one phone...
 
ON the ONS card that this phone runs on, I see one of the 16 lights on the front stays light, it never shuts off.
 
What does the maintenance terminal say for the state of that extension/PLID?
