You can accomplish what you want by creating your document and a "Form" then saving it as a "Template".
To create a "Form" you have to have your "Form Toolbar" open. <View><Toolbars> select <Form>.
Then add the appropriate text and "Form fields". The one step many people forget is to "Lock" the form. There is a button on the "Form Toolbar" that you click on.
Then <File><Save> change <File Type> to <Template>.
If you have End Users who are sneaky you could always password protect the document to prevent any tampering.
It worked perfectly, your instruction was so easy to follow thanks a million. I just have one more question. Is there anyway to lock the text so it won't move when typing?
Example:
First Name ________________ Last Name _________________
Restrict these fields to twenty characters each, so the titles will not move when typing in those restricted name fields?
What I would do is use tabs to set up the first and last name areas. Then insert the "Text Form Field" after each one. You can set the properties of the form field by clicking on the "Form Field Options" button, it will become active once you have placed a field.
You can set the properties of the field from a minimum of 0 to a max of unlimited with regards to text.
If you are not comfortable with Tabs, the form toolbar also has an "Insert Table" button, just make sure not to format any of the lines of the table.
