In a Merlin 820D2 system, when a second CO line / Extension card is existing in the system, but has never been used, does it have to be "placed into service" within programming to begin using it?

You should be able to plug a phone into an extension port and it should work. You may need to program the lines for the phones depedning upon how things were initially set-up.
