loosing D-partition in Win2k Server

I got a strange problem on one of company's server, this is the second time that i lost the lost the D drive partition, and it gives it as Unknown File System. First time i reformat the partition and restore from my backup, and now after like 2 weeks it did the same thing. Its a RAID-5 Setup with 5 IBM SCSI 18.2GB hard-drives and 1 HS. Any input what might be wrong with it?

p.s I heard that IBM recalled a batch of defective hard-drives and that theres supposed to be a utility to identify if the hard drives you are using are in those, but i cant seem to find that utility
