Logging into web manager causes system to hang and reboot

Yesterday I scratched it up as a fluke but today again I logged into web manager, it took a few minutes to load and then phones wen't down again. This is a brand new SD card, 11.0.4.011. What the heck is going on? I would use regular manager but I wanted to update passwords for several people at the same time which web manager lets you do.
 
Okay my web manager log in's may be unrelated or coincidental but monitor shows some critical lines of output (see lines 15 and 16) System has been totally unstable since new SD card.

The OSMemPool error is simply the IP Office running out of RAM, but the ISI Exception may point to a corrupt config or faulty hardware. Try recreating the config, if it continues try recreating the SD card from Manager.

If you can keep Monitor open to try and catch the moment it dies you may be able to see what caused it. Enable pretty much everything in the System tab in Monitor and it should spit something out before it keels over.
