KMODE EXCEPTION...ETC.. Cant re-boot vga or any mode 1

Recently installed second serial port card, has not worked yet. Also, just uUNinstalled old ver. of PCAnywhere, installed new version, then re-booted, however cant get past BSOD. Any ideas on how to re-boot, or what to do. Have tried VGA Mode, Last Known, Original Configuration, Default Configuration, All attempts lead to BSOD !
 
KMODE_EXCEPTION_NOT_HANDLED: An incorrectly configured device driver usually causes this type of error. You can use another section of the blue screen to figure out
which driver is causing the problem.

I am also having this problem and either the ERD didn't work or the server will not boot off the floppy. I tried to repair using the CD but that didn't work either. Any ideas????
 
A year ago, however, easiest solution was to re-install NT 4.0. Problem Solved. Hope you have backups of data!
 
I would have never thought this to be such a common thing but I have just come off the same problem after a rebuild of my NT 4 raid 0+1 setup. Tried to reboot and BSOD with the Kmode error. What we had to do to resolve the problem is (some work but it did resolve the problem) drop files from the SP6 disk into the winnt\system folder and one other (can't remember)from the command prompt (Must find a way to boot there ... hopefully system partition is FAT.) The input of these files allowed me to at least get the machine to a point where I could log in. Once logged init was necessary to reinstall SP6a again because the SP was dropped to SP1 and all shares on the 2nd partition was lost so they had to be re-established ... licenses had to be reset but all data remained intact. Unfortunately I don't have that list of files to place into the directories ...(list is at the office).

If anyone is interested I can post the URL where the MS page with the list of (@ 25) files to drop in from the C-prompt. A bit tedious but it does work and the problem was fixed like a charm ... so far. It has been about a week since it has been up and running again but I unfortunately had a POS crew writing hand tickets on a Saturday while I searched the net and finally put pieces together to come up with this solution. The process should have you up anywhere from 30 minutes to 2 hours (although it took us about 4 hours to find the start of the solution) depending on the type of RAID you are running if any. In our case it was 2 plus hours due to the fact that when we tried to install SP6 once we were able to log in ... the drives were rebuilding while we were installing SP6. Longest install in history of SP6. Hope this info helps and email me 3/11 for file location and drop information.

Guys is is the information I told you to drop into the folders ... this is straight from Microsoft. they also suggest doing parallel installs but if some of you are sketchy about loosing configurations this technique does keep most things intact ... a few installs of programs may have to take place ... for instance we had to reinstall the PAM utility for the promise cards, ethernet cards and the backup software ...

If the boot partition is using the NTFS file system, you can use this workaround if you install a parallel installation of Windows NT on a separate partition or drive, or by moving the disk to another computer to access these folders. If you cannot do this, you can use the procedure that is described in the following Microsoft Knowledge Base article to make repairs:

After you use these steps and log on to the computer, you must reapply SP6A, the SRP, and this hotfix to update the %SystemRoot%\Repair\Setup.log file. After the Setup.log file is repaired, update your Emergency Repair Disk (ERD) by using the Rdisk.exe utility.

All files are located on your SP6a disk...
